Complaints Form 

The Complaint Form is a simple way for you to let us know if something went wrong or if you were unhappy with your experience at Catholic Crosscultural Services (CCS). We are here to listen and address your concerns. By filling out this form, you can share details about what happened, and if you agree, we can follow up with you to resolve the issue. Your feedback is important to us and helps us improve our services. Don’t worry—everything you share will be kept private, and we’ll respond within 48 hours if you give us permission to contact you.

How to Fill Out the Complaint Form

1. Name:

  • Write your full name in the box. This helps us know who is submitting the complaint.

2. How Can We Contact You?

  • Use the dropdown menu to choose how you want us to contact you: Phone or Email.
  • After selecting, a box will appear for you to write your phone number or email address.
  • You must fill out at least one contact method so we can get back to you.

3. When Did It Happen?

  • Type the date and time in the box (e.g., February 5, 2025, at 2:30 PM).

4. Was There Anyone Present?

  • Choose Yes or No by clicking the appropriate box.
  • If you select Yes, another box will appear where you can type the name of the person who was there (if you know it).

5. Where Did It Happen?

  • Write the location where the incident took place. For example, “CCS Mississauga office” or “Online meeting”.

6. Details of What Took Place

  • In this section, describe what happened in as much detail as you can. For example, explain what the issue was, who was involved, and any other important information. The more details, the better we can help.

7. Consent for Follow-up

  • Check the box if you agree to let us contact you about your complaint.
  • If you don’t check this box, we won’t follow up with you, but we will still review your complaint.

8. Submit

  • Once you’ve filled out the whole form, click the Submit button to send it to us.
  • After you submit, we’ll review your complaint and respond within 48 hours if you’ve given consent for follow-up.
